Unlike your traditional skilled nursing facility, Walnut Place provides 24/7 skilled nursing care and services in one comfortable environment, surrounded by the familiar faces of friends and staff. You’ll have the peace of mind that comes with knowing we will be able to meet your loved one’s future needs, no matter the level of skilled care required.
Each skilled nursing neighborhood at Walnut Place provides its own distinct level of care. You’ll find residences for individuals requiring just a little more care than assisted living, as well as skilled nursing care for those needing more specialized attention. You can rest easy knowing all senior care services are directed by a team of Registered Nurses, advisory physicians, and other professionals experienced in long-term care.
The Centers for Medicare and Medicaid (CMS) created the 5-Star Quality Rating System to help consumers, their families and caregivers compare senior nursing homes easily to identify top quality communities and services. Skilled nursing facilities receive a separate rating for health inspections, staffing and quality measures. Communities with 5 stars are considered to have high above average quality.
Walnut Place is proud to continually receive a 5-star rating from CMS. We offer proven post-acute care to help patients recover whenever short-term skilled nursing care or rehabilitation services are recommended. Our goal-driven expertise allows patients to achieve the highest level of function and medical recovery possible to accelerate the transition from hospital to home. Our care-intensive treatment also provides close monitoring of complex chronic conditions or additional pain management as required to those recovering from a serious illness or injury.
Our skilled nursing care staff is highly qualified and has called Walnut Place home for many years. Our long-term team members include wound care certified RN’s, certified dementia professionals, IP nurses and more. We are very proud of our staff members and their ability to connect with our residents. Their dedication and level of support sets our care apart from other nursing homes in the area.
